CONSENT FORM FOR PHOTOGRAPHY & FILMING FOR CHILDREN UNDER 16 YEARS OF AGE,  PARENTS/GUARDIANS, AND PARTICIPANT(S)

By purchasing a ticket to any session(s) under Empowr CIC, and selecting ‘YES’ in the booking form, I consent to be photographed or recorded upon entering the venue (s)  and the use of any photographs and video footage of myself and child/ren are included in my booking. This is for any marketing, publicity and/or fundraising-related purposes, directly related to Empowr CIC without payment or any other consideration.

I understand that the photographs and video footage may be edited, copied, exhibited, published, or distributed in print and on websites, social media, and/or TV, both on platforms controlled by Empowr CIC, and on third-party media.

I acknowledge and agree that:

 

● The photographs and videos will be the copyright and Intellectual Property of Empowr CIC

● Neither myself or my child have any right to royalties and/or any other compensation arising in, or related to, any use of the photographs or videos.

 

By selecting ‘YES’ in the booking form, I acknowledge and agree that I have read and fully understand this consent form, and its contents and agree to be bound by it.
